  Meet our Health Technician Crystal!    Hello Foothill Knolls family, My name is Crystal Syed. I'm your new Health Technician.  Let me tell you a little bit about myself. I'm hispanic, I grew up in Rialto California and I am Bilingual. I have worked for over 20 plus years with the school districts around this area. I have worked in Special Ed and served as a PTA president and treasure for my children's school. I love working with children. I'm very kind and outgoing. I'm so honored to be working here with you all!

Let's make this school year great! If you need anything I'm always available. Thank you all.

Policies & GuidelinesParents/Guardians and Students

Please keep the health technician informed of any special health matters, a change of address and/or emergency telephone numbers. Maintaining up to date information will help in the case of an emergency.

The health office is open during all school hours to assist your child.

Non-Routine Medical Care at School & Medication Policy

If your child requires medical care at school, you will need to provide the school with a written orders from your child's doctor. The school cannot administer medications of any kind, or provide non-routine medical care without these written orders.

If your student needs to have medication administered while at school (this does include over-the-counter medications like Tylenol), please visit the health office for the proper paperwork or download the paperwork here.  Once completed, please return it to the health office and speak with Devon about your student's needs.

Immunization Policy

If you have questions regarding required immunizations, please email or call the health office. Please refer to the sites Shots For School for current immunization guidelines.
